Health Care Benefits:

Medical Coverage  
  • Health Insurance with Blue Cross & Blue Shield.
  • May elect coverage for yourself, spouse, and your dependents.
  • Coverage begins after 30 days of employment.
   
Dental Coverage  
  • Dental Insurance with Mutual of Omaha Insurance.
  • May elect coverage for yourself, spouse, and your dependents.
  • Offered to all employees working more than 30 hours per week.
  • Becomes effective the first of the month following 30 days of employment.
   
Vision Coverage  
  • Health Insurance with Blue Cross & Blue Shield.
  • May elect coverage for yourself, spouse, and your dependents.
  • Receive a 10-20% discount at Doctors Vision Center and Optometric Eye Care Center on glasses, contact lenses, and eye examinations.
  • Offered to all employees working more than 30 hours per week.
   
Long Term Disability (LTD)
  • The Bank provides additional insurance protection against the loss of income when an employee is disabled as a result of an accident or illness.
  • Becomes effective following the first 90 days of employment.
  • The plan will pay the disabled employee 60% of his monthly salary up to $8000.00.
  • Offered to all employees working more than 30 hours per week.
 
Life Insurance
  • Eligible to all employees
  • Amount of Life Insurance is your base salary x 3, to a maximum of $300,000.
  • Accidental Death & Dismemberment Principal Sum is your base salary x 3, to a maximum of $300,000.
  • The benefit amounts above reduce by 35% at age 65 and further reduce to 50% of the original amount at age 70. All benefits terminate at retirement.
  • Amount of Dependent Life Insurance for a spouse is $2,500.
  • Amount of Dependent Life Insurance for child(ren) older than 15 days, but less than 6 months is $250.
  • Amount of Dependent Life Insurance for child(ren) older than 6 months, but less than 19 years (23 year is a full-time student) is $2,500.
